My Transplant-Ready Workbook

The transplant journey can be long, challenging, and overwhelming. To make it a little easier, we are proud to announce the launch of the My Transplant-Ready Workbook: a new resource to help kidney patients (and those who support them) through the entire transplant process.

This Workbook is designed with clear instructions and easy to understand information to help you record important  information as well as understand, reflect and stay motivated through your transplant journey.

The Workbook was created by the Missouri Kidney Collaborative (MKC), committed people from four government-funded and non-profit organizations, who used their combined expertise and experience to develop this resource. It is intended to supplement, organize and build upon the information and guidance provided by medical professionals.

The MKC organizations are: Missouri Kidney Program, The Missouri Department of Health and Senior Services – Organ and Tissue Donor Program, Qsource ESRD Network 12, and Health Literacy Media (HLM).

Printed copies of the My Transplant-Ready Workbook will be available, in limited numbers, at MoKP contracted Missouri dialysis and transplant centers. If you are not able to print out a copy from the website, please ask your dialysis center if you can get a workbook.

Steps to use the fillable PDF:

4. Type responses directly into the light blue boxes on the black lines
5. Don’t forget to consistently Save so that you will not lose your work
